Warning On CimetidineSubmitted by basil on Wed, 2006-10-18 11:57. Cimetidine (Tagamet) is listed as a possible treatment for porphyriai in the Stratton patent. I urge strong caution on taking cimetine as it inhibits the function of the cytochrome P450 1A2, 2C19, 2D6,and 3A4 isoenzyme pathways. These four pathways account for the Phase I metabolism of at least 85% of all pharmaceutical substances, and a large number of endogenous substances as well. Given that many of us already have compromised liver function due to Cpni infection and secondary porphyriai, and are taking large amounts of numerous potent parmaceutical substances requiring Phase I hepatic metabolism, and some of which are P450 inhibitors themselves, cimetidine is essentially poisonous. Careful study of the FDA-appoved physcian's prescibing information for all newer pharmacueticals that have undergone P450 metabolic analysis shows that cimetidine is proscribed whenever one of the above pathways is utilized for metabolism.
(Likewise, grapefruit juice is proscribed whenever the 3A4 pathway is involved, as grapefruit juice is a 3A4 inhibitor. I always advise people to avoid grapefruit juice as well as cimetidine, even healthy people.) In my opinion, the known potential downside of cimetidine is substantially greater than any possible theoretical value in treating porphyria. basil. ___________________________________________________________ »

This makes me wonder as for me, any ingestion of grapefruit juice makes me ill, with a pounding headache. Of course, it may simply be a allergy to citrus. Grapes are also a bit of a problem.

Gracious heavens, Basil! I love grapefruit juice with breakfast and grape seed oil, no matter whether it comes from grape fruit or grapes, makes an excellent light salad dressing mixed with white wine vinegar and a touch of top quality balsamic vinegar, plus crushed cloves of garlic, of course. I think I'll stick with both.......Sarah

Hi basil, My husband Steve has only minor secondary porphyriai which is easily handled with glucose/dextrose before meals and with sublingual B-12. But, since we don't know how bad it might get in the future, and since others on the site have more serious porphyria issues, I just wanted to let you know that I very much appreciate your efforts to unearth and provide this community with so much porphyria-related information. I wish you success in getting to the bottom of your porphyria problem, and I hope the CAPi fixes at least the secondary part of the picture if not all of it.
