My husband is taking Rifampin 600 mg QD and Roxithromycin 300mg BID which would be reasonably comparable to Rifampin and clarithromycin.
Since you list Bartonella in addition to CPNi, it may well take this combo to be effective against the Bart - at least in my research.
All just my personal opinion...

Roxyi 300mg 450mg, Rif 600mg, NACi 1200mg and Tinii/Metroi pulses, clarithromycin 1000mgDoxy, AzithPoland
I'm taking Rif 300mg a day
I'm taking Rif 300mg a day and Clarithromycin 1000mg a day. Can't manage more Rif without the dreadful heartburn. The clarithromycin has definately been eating away at persistent infection in my sinuses.... Otherwise I seem to be coping OK. It has only been a week since I swapped Azi for Clari, so hard to tell what effect it has had until I do a pulse.
If it goes OK I might try adding the other 300mg of Rif.
