Sounds like Kate might be talking to Trevor Marshall. He believes that nobody needs to take any Vit D and that it's "best" to avoid all sources of it in diet and from the sun.
Anyway, Dr Holick describes it quite differently in his findings in that folks with D deficiency develop lots of bone pain as a result of the bones softening as in Osteomalacia [14].
Holick also states that most Drs remember one thing from medical school... to be careful of D toxicity although he said nobody ever sees it because it's extremely rare. I got this info and lots more from this presentation:

I agree with Janice in that if you have a D deficiency (mine was self inflicted, btw) and you raise your Vit D intake, you will most likely experience an exacerbation of symptoms temporarily as in die off. I know I sure did but after you get beyond that point, you will notice the improvements.

Yeah, right. The diet does not provide sufficient D even if you try. I'm living proof it's hard to get enough. I eat fish and dairy and even take a multi-vitamin with 400 iu of D and when I tested I was still borderline deficient at 20 (20-150)....and some labs consider the lower limit to be 30.
And if you live in the northern lattitudes you will not get enough sun most of the year. I'd read that here in WA you could stand outside fully naked for 5 hours in January and you'd get almost zero D from it.
