I am totally confused about the protocolsi now.

Is the doxyi/azith/flagyli combo out-dated?

No, not at all: David is taking it at this very moment!  All the protocolsi listed here are perfectly valid, so don't for heaven's sake start thinking that what you are taking might not work.  I take roxithromycin instead of azith, but they are both macrolides and basically do the same thing.  I have also swapped flagyl for tinidizole, but only because it is more easily tolerated.
Completed Stratton/Wheldon regime for aggressive secondary progressive MSi in June 2007, after four years, three of which intermittent.   Still improving bit by bit and no relapses since finishing treatment.
